Terms and Conditions of Business


This document details the Terms and Conditions of Business of Park House Veterinary Centre.
We reserve the right to alter these terms and conditions at any time.

Opening Hours

Monday to Friday 8.00am – 7.00pm, Saturday 8.00am – 12.00noon.

Consulting Hours

Monday to Friday 8.30am – 7.00pm, Saturday 8.30am – 12.00noon.

Out of Hours Care

Outside normal business hours, emergencies will be seen at Park House Veterinary Centre, 108 Lichfield Road, Stafford, ST17 4ER. Out of hours consultations are charged at £300.00

Fees

All professional fees, food, merchandise and medication charges are subject to VAT at the current rate. Fee levels are determined by the time, equipment, medications, consumables and diets used on a case. An initial consultation is charged at £50.00 and follow up consultations at £45.00. The practice will issue detailed invoices and/or receipts on request.

Payment of Fees

We accept payment by:
Cash
Credit or Debit cards, most major cards accepted with the exception of American Express, Diners Club and JCB.

Estimating Treatment Costs

We will provide written estimates of cost of treatment valid for 30 days. Please note that these are estimates not exact quotations and that every case is individual and will be treated appropriately.
If payment of fees is a concern, please inform the veterinary surgeon BEFORE any treatment has commenced.

Settlement Terms

Fees are due for settlement at the end of a consultation, on discharge of your pet or upon collection of medication or diets.
WE DO NOT OFFER CREDIT FACILITIES.
Should fees remain outstanding, administration and account charges will be levied following our correspondence.

After due notice, overdue accounts will be referred to our Debt Collection Agency and a further charge will be levied in respect of costs incurred in collecting the debt. Any credit card payments not honoured or counterfeit monies will result in the account being restored to original sum plus bank charges and administration costs.

Pet Healthcare Insurance

For those pets covered by pet healthcare insurance please be aware that it is your responsibility to settle your fees with the practice and then reclaim them from your insurance provider. The practice will complete only the relevant sections of insurance forms as by law we are unable to intervene beyond providing clinical notes, costs, and our professional opinion.

Prescriptions

Prescriptions are available from this practice, for which a fee will apply. You may obtain Prescription Only Medicines, Category V, (POM V) from the veterinary surgeon or ask for a prescription and obtain these medicines from another veterinary surgeon or a pharmacy. The veterinary surgeon may prescribe POM Vs only for animals under his or her care. A prescription may not be appropriate if your animal is an in-patient or requires immediate treatment. You will be informed on request, of the price of any medicine that may be prescribed for your animal. The general policy of this practice is to re-assess an animal requiring a repeat prescription every three months; however, this may vary with individual circumstances. A repeat prescription examination will be charged, please ask a member of staff for the current fee. Further information on the prices of medication is available on request. Forty-eight hours notice must be given for all repeat prescription requests. Twenty-four hours notice must be given for all repeat dispensing requests.

Complaints Procedure

We hope that you never have cause to complain about the standards of service provided by Park House Veterinary Centre. However, if there is something you wish to complain about, please direct your comments in writing to the Practice Manager, Mrs. Kathryn Jones.

Ownership of Records and Associated Items

Case records are the property of and will be retained by Park House Veterinary Centre. Copies of clinical histories will be passed on request to another Veterinary Practice taking over the case.
Treatment of your pet may involve carrying out investigations, e.g. radiographs, blood tests, ultrasounds. Even though we charge a fee for carrying out the procedure and interpreting the results, ownership of the record remains with the practice.

Data Protection

We are required under the Data Protection Act of 1988 to keep your personal information for accounting and record purposes. We will never divulge your details to other agencies except for debt collection purposes. We will use the data that we have on file to contact you with information beneficial to your pet's health, e.g., reporting diagnostic test results
If you DO NOT want us to send you such information, please inform the practice manager in writing.

Missed Appointments

We reserve the right to charge for missed appointments.

Variation of Terms of Business

No addition or variation of these conditions will bind the practice unless it is specifically agreed in writing and signed by all Company Directors. No agent or person employed by, or under contract with the practice has the authority to alter or vary these conditions in any way.
